ABOUT THE CLINIC

The 8,000 square foot clinic is surrounded by a previously underserved residential area that continues to experience growth and development of new housing. The Lake Hazel clinic has procedure rooms, lab and digital X-Ray services on site, in addition to a fully staffed Urgent Care facility open every day 8am – 8pm.  The Lake Hazel clinic utilizes EPIC for Electronic Medical Record. 

Support:  24/7 CRNA Anesthesia; Level IV Trauma Center; ED; Saint Alphonsus FM clinic on-site; community family medicine clinics; SAHS specialty physicians rotate in outreach, larger system resources.
